Abstract
Hasta güvenliği uygulamalarında, en dikkat çeken konulardan biri de
hastanın kendi bakım süreçlerine katılımının sağlanmasıdır. Sağlık
sistemlerindeki değişimler, gelişen tıp teknolojisi, önlenebilir hataların bu
gelişmelere rağmen istenilen seviyeye düşürülememesi, hasta hakları ve bu
yüzden sağlık personellerine açılan davalar sonucu; “hastaların tıbbi karar
sürecine katılması” önem kazanan bir alan olarak ortaya çıkmaktadır. Bu
çalışmada, hastaların kendi tedavilerinin ne kadarına katılabildikleri, bu
katılımlarda karşılaştıkları sorunlar ve alınabilecek önlemler belirlenmeye
çalışılmıştır. Joint Commission International’ın geliştirdiği Speak Up
programına ait broşürler, Türkçeye çevrilmiştir. Broşürlerin içeriklerini
yansıtan kırk altı soruluk bir anket hazırlanarak, bir özel hastanede dört yüz
hastaya uygulanmıştır. On bir adımdan oluşan Speak Up programının bu
çalışmada on adımı uygulamaya alınmıştır. Elde edilen verilere göre,
hastaların özellikle, sağlık kurumu seçerken, doktor seçiminde, ameliyat
kararı verirken, eczanelerden ilaç alırken, çocukları hastalandığında çok
daha fazla katılım sağladıkları belirlenirken, hastaların, organ bağışı
konusunda, hasta haklarının bilinmesinde, sağlık personelinin ters ya da
olumsuz davranışında, el hijyeni konusunda, doktorların verdikleri bilgileri
anlayamadıklarında, tedavilerden memnun kalmadıklarında katılım
konusunda çok daha pasif rol oynadıkları belirlenmiştir.
Hekim – hasta ilişkisinde farklı bir etkileşim yapısına sahip olan Speak
Up programı, tıbbi kararların paylaşımı sürecinde yararlı olacaktır.

A Field Study about Patient Participation in
Diagnosis, Treatment and Care Giving Processes; Patient Safety Application
Speak Up Program
ABSTRACT: One of the important topics about patient safety applications is the participation of the patient in the care giving processes. The amount of avoidable mistakes in care giving system does not decrease as much as the degree of evolution of the medical technology. So, participation of the patients in medical decision giving process is much more important than the past. This field study tries to find out how much the patients could attend to these processes, the difficulties being faced, and the precautions that could be taken to avoid mistakes. Using the handouts of the Joint Commission International, a survey is prepared and applied to four hundred patients. Ten of eleven steps are considered through out the thesis work. Analyzing the survey results, it could be concluded that, patients mainly involve in topics like choosing the health service provider, selecting their doctor, giving the operation decision, taking the medicines from the pharmacy or deciding about the problems of their children. However, patients do not participate in the subjects like, living organ donor, knowing the patient rights, understanding the doctors and the other caregivers, hand hygiene or being unsatisfied about the given care. Speak Up will be an efficient way in medical decision giving process, because of the dynamics of the program between the patient and the caregivers.
