| dc.description | ABSTRACT: Health care spending in the world is increasing every day due to the development of medical science and technology. The economic burden mainly comes from high cost of medical services, forcing budget constraints of countries more and more. The ratio devoted to health care budgets of developed countries, can reach up to several times more than the underdeveloped countries' budgets. Now not only the treatment of patients but also preventive health care and protecting people's health must be accepted as a basis. While health care system is planning, these issues should be taken into consideration; especially in institution building, preventive health care for the environment within the system should be more efficient, physical space, vehicles, equipment, personnel for fulfilling the services should not be employed outside the job description. Preventive health services are needed to be on the agenda more and highlighted further when planning health services due to very low initial investment costs, requiring no extra cost during the implementation, because of its contributions to the prevention of environmental pollution by increasing the quality of life areas. In this study, impact of family medicine practice (by this implementation the organizational structure of health services in rural areas are completely changed) to the environmental health services has been identified by using questionnaire to the service-makers. In the study, the situation before and after the implementation was evaluated on the basis of the criterias such as; the physical structure, working conditions, personnel information, environmental health services provided to society and the administrative structure. | dc.description.abstract | Dünyada sağlık harcamaları tıp bilimi ve teknolojinin gelişmesi ile birlikte her geçen gün artmaktadır. Daha çok tedavi edici hekimlik hizmetlerinin yüksek maliyetinden kaynaklanan ekonomik yük ülkelerin bütçe kısıtlarını zaman geçtikçe daha çok zorlamaktadır. Gelişmiş ülkelerin bütçelerinden sağlık hizmetlerine ayırdıkları paylar, geri kalmış ülkelerin bütçelerinin birkaç katına kadar çıkabilmektedir. Artık koruyucu sağlık hizmetlerinin öne çıkması, hastaların tedavisinin değil insanların sağlığının korunması bir esas olarak kabul edilmek zorundadır. Sağlık hizmetleri sistemi planlanırken özellikle idari yapılanma da çevreye yönelik koruyucu sağlık hizmetlerinin sistem içerisinde daha etkin hale getirilmesi, hizmetlerinin aksaklık olmayacak şekilde yerine getirilmesine yönelik; fiziki mekan, araç, ekipman, personelin görev tanımı dışında istihdam edilmemesi hususlarının göz önüne alınması gerekmektedir. Koruyucu sağlık hizmetleri ilk yatırım maliyetlerinin çok düşük olması, uygulanması esnasında ekstra maliyet gerektirmemesi, çevre kirliliğinin önlenmesine yaptığı katkılar dolayısıyla yaşam alanlarının kalitesinin artırılmasının sağlanması yönleriyle artık daha fazla gündemde olması gereken, sağlık hizmetleri planlamaları yapılırken daha fazla öne çıkarılması gereken bir konudur. Bu araştırmada sağlık hizmetlerinin teşkilat yapılanmasını tamamen değiştiren aile hekimliği uygulamasının çevre sağlığı hizmetlerine etkisi hizmet vericilere uygulanan anket çalışmasıyla ortaya konulmaya çalışılmıştır. Çalışmada aile hekimliği uygulamasına geçilmeden önceki ve sonraki duruma ait fiziki yapı, çalışma koşulları, personelle ilgili bilgiler, topluma sunulan çevre sağlığı hizmetleri, idari yapılanma kriterleri baz alınarak değerlendirme yapılmıştır. | |
